NEW YORK
The New York State Insurance Commissioner's office can be reached at: 800-339-1759 , but we're told you must first file online and get a case number.
The website address to file a complaint is: www.dfs.ny.gov.
Once you're on the website, click on CONSUMERS tab near the top. Then click FILE A COMPLAINT.

The New Jersey State Insurance Commissioner's office can be reached at 609-292-5363.
